Meet Our Team

Melissa Uhl – Director of Care

Melissa is the Director of Care for HomeWell Care Services. She has worked at HomeWell for five years and has a culmination of 13 years of experience in providing care for individuals. Her role at HomeWell is to manage and provide leadership to the care managers.  Melissa ensures the business is staying true to its purpose in the community by maintaining high standards for all staff. She is deeply passionate that every staff member views our clients as their extended family members.  Melissa began working in home care because she has a strong desire to see every person treated with the highest respect and care.  She strives to create an environment where our clients know they are not a number, but a part of the family.  Melissa works to accomplish this vision by meeting weekly with every care manager to discuss every client and assist with ideas to best serve them. She plays a big role in staff development where she continually searches and implements training that will be effective for every staff member from caregiver to care manager.  She strongly believes that training is the best way to set yourself apart and produce an environment of trust and respect.  Melissa enjoys working at HomeWell because of the unity with all team members to strive for the best care for all clients.

Jerrika Jaime – Care Manager

Jerrika is one of our care managers. In 2014, she graduated from Colorado State University with a B.S. in Human Development and Family Studies.  She has worked with many different age groups at varying capacities.  Jerrika has worked with HomeWell for over three years and enjoys building strong relationships with all involved in the care process. Jerrika ensures that we are meeting clients’ needs by visiting with clients in their homes to set up care, creating individualized, detailed care plans, and forming a care team that best fits each client. Her goal is to help create an environment where client/family/caregiver feel valued and secure. Jerrika feels that communication is a pivotal piece to this practice and that when all parties work together for the good of the client, joy can be present despite the challenges that arise.  She truly enjoys being a small part of bringing enrichment in the lives of our clients and is truly thankful to be a part of the HomeWell team.

Collen West – Care Manager

Colleen is one of our care managers.  She has over 35 years of management, team building and is an expert in customer service. Colleen has spent countless hours in community outreach with various organizations.  She has worked with HomeWell since 2017.  Colleen meets with families to discuss the level of care needed and to build a care plan that is customized to each individual client.  She works closely with our team of caregivers to ensure they are exceeding the expectations of our clients by regularly conducting quality assurance visits with current clients.  Colleen comes from a long history of family caregivers and chose to work in the home care industry because her grandmother was a live-in caregiver and her mother cared for many senior neighbors in their homes and sometimes in the house she lived in as well.  Through growing up in this environment, she found that she loved making a difference in people’s lives and advocating for others. Colleen was a salon owner for over 35 years and created teams of stylist to do outreach in the community with battered women’s shelters, women’s rehabilitation centers and homes for unwed moms.  Her entire life has been about helping others and improving their quality of life and this position fits her passion. Returning to the senior population is like coming full circle for Colleen, she loves helping them find joy and quality of life.

Cassandra Villa – HR Manager

Cassandra is the Human Resources Manager at HomeWell Care Services. She is excited to have recently joined the company in October.  She has 15 years of administrative experience and 5 years of Human Resources experience. Her most recent experience in HR has been in benefits, employee relations, on-boarding/termination of employees, recruiting and reporting. Cassandra enjoys spending time with her family dirt biking and camping.  She loves watching her 4-year-old daughter explore and learn. For Cassandra, joining HomeWell Care Services means she gets to do what she loves best which is working with employees who all strive to make a difference in the lives of others. She also enjoys being the face everyone is comfortable talking too. She is excited to be part of the HomeWell team and is looking forward to growing her career and facing new challenges.

Harley Freeseman – HR Admin Assistant/ Scheduling Coordinator

Harley triples as a caregiver, on call coordinator, and office administrator. She is outgoing, caring and loves people. Currently she has plans to finish her degree at Front Range to become a nurse. Harley has always loved building relationships with the people around her. This makes her passionate about all the work she does with HomeWell. She loves working with our seniors because they always have something new for her to learn, and she loves being able to help others live comfortably in their homes, it is extremely rewarding.

Senite Tewahade – Schedule Coordinator/ Billing Specialist

Senite works as a schedule coordinator and billing specialist.  She strives to treat clients as people rather than dollar signs. She began as a caregiver at HomeWell four years ago and quickly learned the true value of homecare. It was a setting that gave her the ability to provide quality care and true companionship to seniors. This was ultimately the reason she began, and remains, in homecare. As a schedule coordinator, Senite works to match clients with caregivers to ensure the best possible care. In addition to coordinating care, she acts as the Department of Veteran Affairs billing specialist. She enjoys this role because it allows her to give back to the veterans HomeWell serves.

Wendy Stephenson –  Northern Colorado Community Outreach Manager

Wendy is our Community Outreach Manager for Northern Colorado. She holds a B.S in Business Management and comes to us with over 20 years of experience in healthcare. For the past 14 years, Wendy has had various healthcare sales and marketing roles including those in skilled nursing, assisted living, memory care, home health and hospice. Building and fostering relationships within the community is her passion along with working with seniors. In addition to her professional roles, Wendy has served her communities as a victim advocate for domestic abuse, volunteered in her children’s the schools and helped with various organizations to serve the homeless. In her spare time, Wendy enjoys traveling and spending as much time with her family as possible. She is excited to be a part of the HomeWell family and promote our presence in the community.

Sandra Melena – Care Manager

Sandra started in the Healthcare Industry in 2002 as a CNA. It was at that point that she had realized how much she enjoy taking care of others. Sandra has worked in many health care settings as a CNA and has enjoyed every bit of it. She believes being a caregiver is in one’s heart, it is not just a job. Over a year ago, Sandra made the change to work on the administrative side of caregiving. After so many years of working in the field, her body decided she could no longer work as a CNA. Sandra did not want to leave healthcare, so at that point she became a Care Coordinator, and loves it! She was now in a position in which she can still make a great impact in someone’s life, and make sure that they are receiving the best care from the caregivers!

HomeWell assigns each client a dedicated Care Manager whose sole purpose is to ensure the highest level of customized in-home care to meet your needs and your budget. Your Care Manager will initiate a comprehensive needs assessment and create a personalized plan regarding:

Once a plan is in place, your Care Manager will monitor daily care logs and maintain regular contact with everyone involved to ensure each client’s physical and emotional well-being. And because life can be unpredictable, your Care Manager can also recommend or make adjustments to your care plan at any time, as needed.
